Home Care

Home care provided by Southeastern Wake Adult Day Center is supportive care delivered in the home by professional caregivers. Home care addresses activities of daily living (ADLs) and is vital for frail or disabled adults who have a desire to remain independent and living at home.

Program Offerings Include

  • Food preparation and assistance with eating and drinking

  • Personal care including bathing, grooming and dressing

  • Monitoring vital signs such as blood pressure, temperature, heart rate, and breathing

  • Medication Reminders to ensure medication is being taken as prescribed

  • Health monitoring, including pain assessment and daily weight checks

  • Light housekeeping

  • Other duties as outlined in the plan of care

Southeastern Wake Adult Day Center’s home care services are offered by licensed, trained, Certified Nursing Assistants (CNA) under the supervision of a Registered Nurse in accordance with the North Carolina Nursing Practice Act. The Registered Nurse works with the client and their medical providers to develop a daily plan of care for the CNA to follow while in the home. The plan will include:
